I thought at first maybe sacks, but that's not too out of line and doesn't explain the awful short passing stats. I think the main culprit is probably too much 113 personnel usage. I'm betting it is familiarity killing your offense.
The rule of thumb I use for formations is as follows:
113: No more than 60% of my game plan.
122: No more than 30% unless I am running an erhardt-perkins scheme.
212/104/203: max 8 plays from any one of these formations, almost never using the same play twice in a game plan. These are the tertiary formations, very rarely are any as important as 122 and never as important as 113. Some schemes will allow for more use of one of these.
131/221/023/014/005/230: No more than 3-4 of any of these formations. Never run the same play in two sections of the game plan. These are mostly gimmick formations that aren't able to be heavily used as far as I know. I think a spread team could get away with more 005 and 014 stuff and a Erhardt Perkins team could probably run more 023 than usual, while a smashmouth plan can lean more heavily on the 221, etc.
Now I don't know how similar my ideas are to anyone else, but this has helped my offenses a lot.

substituted QB -- didn't change anything and JUNE was outperformed pretty handily by the backup qb
